How can I obtain copies of medical records from previous visits to St. Luke's Health?

To protect our patients' privacy, we don't transmit confidential patient information via e-mail. The best way to obtain copies of your records is to call our Medical Record department at (713) 948-7023 during business hours, Monday - Friday (8:30 a.m. - 4:30 p.m.). Our representatives will be happy to assist you.

To obtain a copy of your medical records, you will need to complete this form (form in English | forma en Español).You can deliver the form in person to a St. Luke's Health Medical Records Department located at one of our  hospitals, or you can mail it. IMPORTANT: There may be a charge for your records, so we strongly encourage you to call and speak to one of our representatives before mailing in the above form.
